Carnifexes are not meant to have names or histories; they are disposable engines of destruction grown for a single campaign and left to rot on whatever world claims them. Old One Eye should have followed that same forgotten path decades ago. Instead, this particular carnifex, first encountered and marked by a scarred, ruined socket where one eye should be, has been reported active across multiple hive fleet incursions separated by years and vast stretches of the void.

Imperial scholars remain divided on how this is possible. Some argue the Hive Mind has deliberately preserved and regrown the creature's genetic template each time it falls, effectively resurrecting the same beast again and again because its survival instincts and combat record proved too valuable to lose. Others suggest something stranger: that Old One Eye has developed a rudimentary self-preserving will of its own, an anomaly the collective intelligence tolerates rather than commands.

Whatever the truth, soldiers who have fought it more than once describe an unmistakable, bone-deep dread at recognizing that ruined eye socket charging toward them again, as though death itself had simply declined to keep its bargain the first time.
